This form is for your school’s information. We will use this information to keep our website updated and accurate so that students will be able to efficiently contact you for your courses. We ask that you designate one person as the sole contact (or at most two, one for undergraduate and one for graduate) for us and prospective students. An administrative assistant, who is available daily, would be preferable.

Once we receive your information we will contact you for billing and confirmation. There are also .pdf versions of these forms to print out in case you want hard copies.
